Affidavit of Facts (AOF)
  • A report needs to be filed with the Police Department for the incident.
  • If the charges are a Class A & B Misdemeanor where no arrest was made at the time of the event, the Grayson County District Attorney requires that an AOF be downloaded and completed by the victim in order to file the charges. 
  • The AOF is a sworn statement by the victim that requires a notary to witness the victim's signature at the bottom of the back page.
  • A notary is available to assist you at the Police Department Records Division, Monday through Friday, 8 am to 5 pm.
